REQUIREMENTS AND APPLICATION FOR MEMBERSHIP Applicants for membership must have met the following requirements: 1. Must be a Filipino citizen. 2. Must have attained legal age. 3. With capacity to contract. 4. Possesses a common bond of association. 5. Resides, works, or has business and other interests in the area of operation. 6. Has completed the prescribed Pre-Membership Education Seminar. 7. Has submitted a completely filled-up Membership Application Form (MAF) 8. Has paid the following: - Membership Fee: Php 100.00 - Share Capital: 1,000.00 (Common for Regular/Preferred for Associate) - Savings Deposit: 555.00 - Membership ID: 100.00 - Annual Due: 30.00 - Others: Damayan: 400.00 Plastic Holder: 15.00 ATM MyPitaka Savings: 200.00 Total: 2,400

1. This is in lieu of the old ATM card. Acceptance of application started on November 16, 2017 2. Non-interest bearing Savings Deposit. 3. Can be used anytime, anywhere thru Bancnet accredited banks. 4. No minimum balance required. 5. Available for P100.00 only.

What are the Membership Classifications?
Answer(s):
1. New Member - refers to a member whose length of membership has not reached 6 months yet. 2. Member In Good Standing (MIGS) MIGS are classified into 5 categories: 2.1. Nickel - at least 6 months in membership and has complied the following at the beginning of the prescribed membership period: has at least P1,000.00 share capital; has at least P500.00 daily maintaining balance on savings deposit; and with a quarterly transaction of any amount; 2.2. Bronze - at least 2 years in membership; has at least P5,000.00 share capital; has at least P2,000.00 daily maintaining balance on savings deposit; and with a quarterly deposit of at least P500.00; 2.3. Silver - at least 3 years in membership; has at least P10,000.00 share capital; has at least P5,000.00 daily maintaining balance on savings deposit or time deposit; and with a quarterly deposit of at least P1,000.00; 2.4. Gold - at least 4 years in membership; has at least P15,000.00 share capital; has at least P7,000.00 daily maintaining balance on savings deposit or time deposit; and with a quarterly deposit of at least P1,500.00; 2.4. Platinum - at least 5 years in membership; has at least P20,000.00 share capital; has at least P10,000.00 daily maintaining balance on savings deposit or time deposit; and with a quarterly deposit of at least P2,000.
